Healthcare Estate Decarbonisation: The Role of Heat Networks
As the UK looks to transform and decarbonise its energy generation to meet net zero carbon targets, heat networks have a vital role to play, the application of this concept lends itself to the typical campus nature of healthcare estates.
District heating in broad terms is a decentralised heating network, the presentation will aim to sumerise how modern heat networks are an efficient means to provide heat to end users in place of more traditional stand alone boiler systems, less efficient technologies and how this can be applied into the health sector. With the introduction of the Energy Act 2023 there are likely to be implications for healthcare estates should they fall within an identified heat network zone.
The transition to heat networks means we must embrace a wider, more coordinated approach with the wider community relative the to the use of heat, we must also align this thinking with existing energy infrastructure and target greater, more deliverable economic and carbon efficient outcomes.
Campus style estates lend themselves to scaled networks, with large thermal and electrical loads, there are clear opportunities for the development of schemes with flexibility that could enable future changes in heat sources to be accommodated.

The Challenges of Going Green – a Hospice Perspective
Hospice Care provision and getting the the balance between the finances, workforce and net carbon zero agenda right can prove challenging. Especially considering the additional pressure of being a charity (Overgate is only partially funded by the NHS). The Hospice is an anchor institution in the local area, as a charity we depend on our supporters to raise the £3m a year to keep our services running, As such we have an obligation to ensure we are committed to the social value agenda.
The Hospice estate is varied, consisting of the Hospice building, offices, 16 charity shops, a distribution centre and a new capital build for an extended in patient unit, which means the scope is vast. This means it can be difficult to know where to start, we are very lucky as one of our supporters is a leading green solutions provider and has undertaken a digital clipboard exercise to understand our current position and the scope we have for improvement. From this we have taken the quick-wins and now are working through those actions where the investment saves the Hospice money.

1. More Than a Hospital: Catalysing Urban Regeneration
MMUH is more than just a healthcare facility; it is a catalyst for the urban regeneration of Smethwick and the surrounding areas. This project leverages a significant investment to revitalize the local community, enhancing economic activity, improving public spaces, and fostering a sense of community. Our masterplan includes green developments, improved connectivity, and public spaces aimed at encouraging social interaction and economic growth.
2. Modern Design: Standardized Grids and Prefabrication
To achieve an efficient, flexible, and legible building, we employed modern design principles including standardized grids and prefabrication (facades, roof, services). The use of a 7.8m x 7.8m grid system throughout allowed us to remove transfer structures, streamline the construction process, maximize usable space, and create a structure that adapts to changing needs. This approach not only improves efficiency but also ensures that the hospital can evolve with advancements in healthcare
3. Beyond the Ordinary: Creating a Unique Healthcare Environment
MMUH breaks away from the typical NHS hospital design. Central to this is the 4,000m² winter garden, a flexible, covered space that serves as a hub for hospital activities, from daily interactions to special events. The overall design integrates natural light, open spaces, and a calming environment, creating a healthcare setting that promotes well-being and reduces stress for patients and visitors alike

Digital Technology to Aid Wellbeing in Hospitals and Elderly Care
The use of digital technology enables us to easily change a landscape and soundscape affecting not just what we see and hear but our biological and emotional selves so affecting how we feel and react in a space.
Starting with our sensory selves and our ideas of comfort and acknowledging how this can differ for different people and with ill health, this presentation shows how digitally adapting and personalising light, view and sound within a healthcare space has been shown to positively affect both patients and staff
Involving adaptable biocentric lighting and themed soundscapes, alongside personalised sound tracks and views we show how digitally adapting our interior environments can be used to enhance our patient experience in many ways. From enhancing appetite, improving sleep quality, and rest, through to increasing alertness and improving performance. This helps people feel more settled, and comfortable so reducing anxiety and tension.
From the user perspective all this is available at the touch of a button or screen

Eastbourne Hospital Renewable Energy Project
Eastbourne District General Hospital is the first hospital in the United Kingdom to be exclusively heated by renewable energy. In a landmark project the Trust, in collaboration with the Carbon and Energy Fund (CEF), bid for and won £28million from the third phase of the Salix Public Sector Decarbonisation Scheme to deliver an ambitious energy overhaul of this 1970s acute hospital. The project had two key aspects; significant overhaul of the entire building fabric and conversion from steam to a multistage air source heat pump system, supported by solar photovoltaics. Ahead of project commencement, the Trust on recommendation from the CEF installed heat meters on every heating circuit in the hospital, which for the first time ever has enabled the impact of a holistic approach to thermal improvement to be quantitively appraised. This presentation will present the data analysis to date, which is anticipated to show a much improved case for holistic fabric improvements compared to a piecemeal approach that may influence future capital spend on energy projects.
This presentation will provide the latest summary and learnings from this landmark project and an opportunity for questions and discussions with the presenters.

Enhancing Patient Experience in Waiting Areas Through Holistic Design and Aesthetic Integration
The experience of waiting is a significant yet often overlooked aspect of healthcare delivery. Patients frequently spend considerable amounts of time in waiting areas, with average waiting times for NHS services extending significantly and durations in waiting rooms ranging from one to six hours. The design of the waiting spaces plays a crucial role in shaping patients' overall healthcare experience, impacting their perceptions of care, emotional well-being, and subsequent interactions with healthcare professionals, which can subsequently affect treatment processes. The role of art as a positive distraction in waiting areas has been acknowledged, yet the impact of the holistic interplay between art and other elements of the physical environment remains underexplored.
Aims: This study aims to bridge the existing research gap by adopting a holistic approach to waiting area design in healthcare facilities. It seeks to evaluate how various aesthetic elements of design contribute to improving patient comfort and the overall experience.
Methods: A mixed-methods research design is employed, beginning with a systematic literature review to gather insights into previously studied aesthetic elements of waiting area designs. Subsequently, a quantitative study is conducted using patient questionnaires to assess preferences regarding different design elements, including art, nature, window views, etc. Participants will be asked to compare and select their most preferred designs from a range of waiting area configurations. Additionally, interviews will be conducted with patients in two waiting areas featuring various design approaches.
Results: Preliminary findings indicate a strong preference among patients for waiting areas that incorporate both art and elements of nature, as well as providing design configurations for privacy and safety. These preferences underscore the importance of a holistic design approach that considers both aesthetic appeal and functional practicality.
Conclusions: The study concludes that the design of healthcare waiting areas should not be overlooked, as it plays a vital role in patient care. A well-designed waiting area can mitigate the stress and anxiety associated with waiting, improve patient-doctor communication, and influence treatment outcomes. The research highlights the need for healthcare facilities to adopt a holistic design strategy that integrates both aesthetic and physical design elements to create environments that support patient well-being.
Implications: This research will have potential implications for healthcare administrators, architects, and designers, offering a framework for the development of waiting areas that cater to patient needs and preferences. This study advocates for a shift in perspective, recognising the waiting area as a crucial component of the healthcare environment that warrants careful consideration and strategic design.

IHEEM Strategic Estates Management Advisory Platform SEM AP) has now been running for 18 months. Its overarching purpose is to provide guidance and best practice for the strategic end of healthcare projects and system-wide issues.
It has four subgroups that have been very active over the 18-month lifespan:

NHSE Policy View on the Digital Agenda
Digitalisation of the NHS Estate is a key priority for NHS England. During this session, the adoption and integration of digital technology solutions will be explored alongside how these solutions can accelerate transformation, add value, optimise space, and improve the overall patient and staff experience.
The session will highlight NHS England’s plan and status to shift to digitally enabled technology. A case study example will demonstrate activity to date and opportunities available for wider geographical spread across the NHS Estate. In addition, an update will be provided on the development of an NHS estates digital maturity matrix which will enable organisations to map their path to smarter buildings and interoperable data.

Aiming for Net Zero in Operating Theatres
The presentation will report on the outcome of a large multi-disciplinary study which was carried to support the Estates teams at two NHS Trusts (UCLH and Barts Health) in looking at the efficiency and energy reduction of hospital theatres. Using support from UCLP through the Greener NHS initiative, the team worked on two activities
1. Rethinking the Operating Theatre Suite: led by Dr Anne Symons UCL (Healthcare Architect)
2. Operating Room Ventilation: Turn it down, Switch it off: led by Professor Ian Eames UCL (Mechanical Engineer).
The aim of Project 1 was to be able to select suitable operating theatres in the trusts where trials could take place. It involved visits and studies to various hospitals within the trusts and in Germany and the Netherlands to study not only the ventilation systems but to see what effect the layout of the theatre suite had on net zero and saving energy. Project 2 supported by Professor Duncan Wilson, CASA, UCL and student teams working with Professor Ian Eames, collected and analysed data in relation to the ventilation systems through sensing the systems and taking information from BMS Systems. The academic team were supported by a medical team led by Consultant Paediatric Anaesthetic Consultation Dr Jonathan Groome, Barts Healthcare and GASP (Green Anaesthesia) whose aim was to reduce energy costs within Barts Health. Lindsay Muirhead from UCLH Estates Department led the sustainability team.
All estates teams in the participating hospitals provided expert technical information that permitted the team to examine the efficiency of the theatres from the perspective of (a) how the space is used, (b) how the space is configured, and (c) the type of ventilation. Three types of ventilation: Traditional – Turbulent Flow, Ultra Clean (Laminar Flow) and Temperature Controlled (Avidicare :Opragon). Currently temperature-controlled ventilation is not included in HTM 03 Specialised Ventilation. The presentation will report on energy estimates and savings, how to use BMS data for energy costs, and share the best practice and experiences that already exists across the NHS estates teams.

Whilst it seems everyone is gearing up for and thinking about PFI, PPP or LIFT hand back, what if you are still in the early or mid concession stages, with years left to run on the operational contract?
This presentation will focus on how to get the best out of your PPP /PFI arrangements over its remaining life, whilst not prejudicing any handback considerations.
So much may have changed since your buildings were handed over – the services you deliver, how you deliver them, the needs of those you deliver too, your workforce and their preferred ways of working… the list could go on.
You are probably therefore considering how to ensure the estate continues to meet your needs , whilst perhaps feeling wary of re-opening those debates and discussion you remember from when the deal was originally made. Or perhaps you weren’t around at the time and don’t know what to expect.
